Not sure if I had to fill out the same form. We're getting married in a Catholic church so had to both show baptism certificates, get letters from all parishes lived in saying we're free to marry (H2b got a letter from husband dad cos he's Methodist), then had to give the certificate to show we had attended the marriage preparation course. I had to sign a form to say I will continue to practice as a Catholic(H2b had to agree to this) then the paperwork was sent to the bishop to grant dispensation for me to marry a non Catholic. Paperwork is in the church system at the moment.
We didn't need to show any ID to the priest. I would presume that you may need to produce say a passport or driving license ancient maybe baptism certificates?
Can you cal your priest and ask what it is he needs from you?

Had our dream perfect honeymoon in Hawaii!
The bishops house thing is just the guidance given on things like getting married outside of your local parish (and the qualifying connections you need to have to get married in the church) and things such as remarrying. If you google it, you can download it in PDF format xx
